What is perineal rehabilitation?

Perineal and pelvic rehabilitation is treatment intended to strengthen the pelvic floor. Strong perineal muscles improve pelvic health and reduce the risks of a leaking bladder. Pregnancy and childbirth exert great amounts of pressure on the pelvic floor which in turn may weaken it and lead to problems.

The perineum, also referred to as the pelvic floor, includes all the muscles, ligaments and membranes that support the organs in the pelvis like the rectum, the bladder and the vagina for a woman. Men too have a pelvic floor and this includes the rectum, the scrotum and only a portion of the urinary tract.  As well as providing support for the organs in the pelvis, the perineum has many other roles. First of all it helps ensure that the sphincter works correctly as well as retaining urine, feces and gas. The perineum also supports normal sexual functions meaning orgasms for women and erections for men. The perineum provides support for the lumbar region as do the abdominals.

Why do perineal rehabilitation?

Perineal rehabilitation is strongly recommended for women after pregnancy and who have had a vaginal delivery. Pregnancy and childbirth stretch out muscles that have difficulty returning to their normal state and tonicity. This is even more true when woman gain a lot of weight during pregnancy, if labour was long or when woman have several pregnancies back to back without using their perineum sufficiently in-between them.

When the pelvic floor muscles and connective tissues are weak and not able to tighten enough, women may experience loss of urine when coughing, sneezing or during an intense emotion. The weakening of these structures leads to a loss of control over the sphincter that relaxes with ease. The person might also have trouble controlling defecation or passing gas.

Moreover, the perineum plays an essential role in normal sexual function. A weak pelvic floor can affect sexual intercourse and even reduce its quality. In some cases this weakening can lead to pelvic organ prolapse and pelvic organ descent.

What does perineal rehabilitation entail?

It is very important to strengthen the perineum as quickly as possible after childbirth to avoid any possible problems. Perineal rehabilitation is conducted with a specialist who uses manual techniques for you to become aware of your pelvic floor muscles. Different equipment can be used such as vaginal probes that are used to induce vaginal contractions. Your specialist will also advise you as to what exercises to do at home to tighten your pelvic floor muscles on your own.
